Which Kerala lottery claim form do you need?

You need Form V for any prize above ₹10,000, plus Form VI, VII or VIII by case. A prize up to ₹10,000 uses a short application instead. The Directorate’s prize claim page, updated 24 March 2026, sets these rules.

The rule numbers are printed on each form. They refer to the Kerala Paper Lotteries (Regulation) Rules 2005, as amended in 2024. The Kerala Paper Lotteries (Regulation) Amendment Rules 2024 set out Form V, the joint claim and the outside-Kerala affidavit. The Directorate publishes the rules as amended as a PDF.

What is Form V, the Kerala lottery prize claim form?

Form V is the 2-page application every winner files to claim a prize above ₹10,000. It records the ticket, the winner and the bank account for payment. It ends with a declaration and the winner’s signature.

Form V has 5 parts, lettered (a) to (e).

  • Part (a): the draw name and number, and the ticket number.
  • Part (b): the mode of presentation, in person, by post or through a bank.
  • Part (c): the winner’s name, address, phone number, PAN and Aadhaar number.
  • Part (d): whether the winner is a Kerala lottery agent, with the agency number.
  • Part (e): the bank, branch, account number and IFSC where the prize money is deposited.

How do you fill in Form V?

Fill in Form V with the name, address and account details on your Aadhaar card and passbook. The Directorate checks them against the copies you attach.

  1. Download Form V from the Directorate’s download forms page.
  2. Write the lottery name, draw code and full ticket number, such as KR-770 and KF 335755.
  3. Tick the mode: in person, by post or through a bank.
  4. Enter your name, address, phone, PAN and Aadhaar number as on Aadhaar.
  5. Mark “No” in part (d), unless you are a registered agent.
  6. Enter the bank account and IFSC for the prize money.
  7. Sign the declaration that the reverse of the ticket is filled in.

What is the “If through bank” part of Form V?

The “If through bank” part names the bank and branch that present your ticket to the lottery office. The Directorate tells winners to leave it blank for a claim in person or by post. A bank claim adds the authorisation on page 2.

That authorisation lets the Director of State Lotteries pay the prize to the named branch on demand. Form V calls the authorisation irrevocable. The bank route works for every prize above ₹10,000.

What is Form VI, the joint claim form?

Form VI is a sworn affidavit for 2 or more people who bought one winning ticket together. Each winner files a separate Form V. The group files one Form VI, attested by a notary.

Form VI holds a table with 1 row per winner. Each row gives the name, address, Aadhaar number, bank account and IFSC, PAN, Form V number and percentage share. The winners agree to statutory deductions in proportion to their shares.

The winners accept joint and several liability for any future claim on the prize. The notary adds a name, seal and registration number, according to the Directorate’s prize claim page.

What is Form VII, for winners from outside Kerala?

Form VII is a 1-page affidavit for prize winners from outside the State of Kerala. Form V treats a winner as outside Kerala by the Aadhaar address. The winner files Form VII with Form V and all the standard documents.

The affidavit states 5 points.

  • Address: the permanent and present address, with the Aadhaar number.
  • Ticket: the lottery, series and number.
  • Source: the ticket was bought from an agent or seller of Kerala paper lottery tickets.
  • Prize: the prize won and the draw date.
  • Responsibility: the winner alone answers if the claim is later found illegal.

A notary verifies it with the Aadhaar and adds a seal and registration number. The claim then goes in person, by post or through a bank. Kerala lottery tickets are sold only in Kerala, so the ticket itself comes from a Kerala seller.

What is Form VIII, for torn or damaged tickets?

Form VIII is an affidavit for a winning ticket above ₹10,000 that is torn or damaged. It cites Rule 9(9). The winner swears it before a notary and submits it with the damaged ticket.

In Form VIII, the winner affirms 4 facts.

  • Ownership: the winner is the rightful owner, and the ticket came from an authorised agent.
  • Damage: the ticket was damaged by accident, on a stated date.
  • Genuineness: the ticket was obtained by legal means and is genuine.
  • Refund: the winner repays the prize in full when the ticket proves fake or ineligible.

Form VIII does not guarantee payment. The office examines the ticket, and tampered or damaged tickets are refused, according to the Directorate. The steps for torn and lost tickets are on the damaged tickets page.

Which form is used for a Kerala lottery prize up to ₹10,000?

A prize below ₹10,000 at a lottery office needs an application for prize payment, not Form V. The Directorate’s claim page lists 4 items for it. An authorised agent pays small prizes without these papers.

  • Application for prize payment.
  • Original winning ticket, with your name as on Aadhaar and your signature.
  • Self-attested copy of the Aadhaar card.
  • Self-attested copy of a single-account bank passbook, with IFSC.

The download page carries a public prize voucher in Malayalam. It is a receipt for prize money paid at a District or Sub Lottery Office. The winner gives the name, Aadhaar number and address, and signs over a revenue stamp.

Which documents go with the Kerala lottery claim forms?

Form V goes with the signed ticket, a passport photo, and self-attested passbook, PAN and Aadhaar copies. That is 6 items in all, according to the Directorate’s prize claim page. Your Aadhaar and PAN must be linked.

  1. Sign the back of the original ticket with your name as on Aadhaar.
  2. Fill in and sign Form V.
  3. Attach 1 passport-size photograph.
  4. Attach a self-attested passbook copy with name, address, account number, branch and IFSC.
  5. Attach self-attested copies of your PAN card and Aadhaar card.
  6. Add Form VI, VII or VIII when your case needs it.

Form V asks for the photo and copies with the ticket. This applies when the form is not filed through a specified portal. The Directorate’s claim page names no public portal for this.

Where do you submit the Kerala lottery claim forms?

You submit the forms at the lottery office that sanctions your prize amount, or through a bank. The Kerala Paper Lotteries (Regulation) Amendment Rules 2024 set the limits. A bank can present the claim at any amount.

A weekly 3rd prize of ₹5 lakh or ₹10 lakh goes to a District Lottery Office. A ₹1 crore weekly first prize goes to the Deputy Director. A ₹12 crore bumper prize goes to the Director.

How many days do you have to submit the claim forms?

You have 90 days from the draw date to surrender the ticket with the forms. The Directorate’s claim page and every official result sheet print this limit. The 2024 amendment replaced the 30 days of the 2005 rule text.

A ticket from the KR-770 draw of 3 October 2026 is claimable until 1 January 2027. Prepare Form V early, since Forms VI, VII and VIII need a notary visit.

Download the forms from the download forms page of statelottery.kerala.gov.in, the Directorate’s website. Each form opens as a PDF. The page was last updated on 24 March 2026.

  2. Find the row for Form V, VI, VII or VIII.

  4. Print the form and fill it in.

The same page lists 7 more forms, the public prize voucher among them. The other 6 are the agent nominee form, 2 agent vouchers, the demand form, and Forms III and IV. Forms III and IV serve lottery agents, not prize winners.

This site does not host the PDFs. The Directorate updates them, so a copy saved elsewhere can be out of date. The Directorate’s prize claim page lists the documents in Malayalam.

Do the Kerala lottery claim forms need a notary?

Yes, for Forms VI, VII and VIII, but not for Form V. Each of the 3 affidavits ends with a space for the notary’s name, seal and registration number. Form V needs only the winner’s signature.

The 2005 rule text asked winners from outside Kerala to have every document attested by a notary. The current practice replaces this with the single Form VII affidavit.

Can you submit Kerala lottery claim forms online?

No. The original paper ticket is surrendered in person, by post or through a bank. Form V mentions a specified portal, but the Directorate’s claim page names none for winners. No website or app claims a prize for you.

The forms themselves cost nothing to download. Kerala Police warn that demands for GST, stamp duty or a fee to release a prize are scams. Report them on helpline 1930.
